SPORTS PHYSICALS!!!!!
Tuesday, June 7 (Last Name A - M) & Wednesday, June 8 (Last Name N -Z)
we will be having our free sports physicals from 6 pm - 8 pm.
Please come to the front of the "H" wing with a completed Athletic Packet. I have
attached the packet. Please fill out the first 3 pages completely and the Doctor fills out the 4th page. You cannot participate in any sports at Linden High School without a sports physical.
